In
1988 F. Dubois in Lyon, France, performed the first laparoscopic
cholecystectomy. In the last 15 years, laparoscopic surgery has
become increasingly more important. Since that time some urologic
surgeons have accepted this new technique, but the indications
for laparoscopic surgery in Urology have been limited and the
technology still needed a significant amount of improvement. This
new surgery was implemented only in a few urologic centers of
excellence. With more experience and with refinements in technology,
laparoscopy has become the focal point in urology and in the near
future will undoubtedly substitute many operations that had been
performed by classic open surgery.
Surgical Techniques
Through a small needle, inserted in the peritoneal cavity, a harmless
gas (CO2) is introduced until the abdominal wall is distended.
It is then possible to insert a port of 5 or 10 mm into which
an optical device is introduced. A TV camera is connected and
the peritoneal cavity is explored.
Under visual control more 5 or 10 mm ports are introduced. Trough
these ports surgical instruments are introduced
like scissors, forceps, retractors, aspirators and the operation
is performed with the same surgical technique
as in open surgery
Operations
Numerous operations are performed today in the Urologic Laparoscopy
Center with the advantage for the patient of a short hospitalization
and convalescence.
